Slim body vs. a face with volume

There are many different kinds of diets that are being introduced that claim to give you a slim body and a lot of people today are interested in weight control. Successful dieting can help give you a slim, supple body, but it can raise another concern - that is, too much loss of fat around the face. This can result in your face making you appear older than your actual age.
Not long ago, Korean celebrities with a friendly look and a gift for gab were bandied about with their big transformation into what is locally referred to in Korean as "momzzang" (Killer Body). But what drew more attention than their muscular build was their rapid aging of their face. Wrinkles became more noticeable and their faces were drawn from losing too much weight, which even made them look a bit cringeworthy. Moderate volume in cheeks and forehead with no hint of facial bones makes a person look young and polished.
In the previous article, I talked about the craze for younger looking faces and being faced with a choice between having a slim body and full face with volume would put you in a dilemma.

Face of beauty = younger looking face

Unlike Westerners wanting to have a sharp look with angular jaws and cheek bones, Asians have a desire to have well-rounded, curved faces. This is further proven with Asian female celebrities who are considered beautiful. But in order to have such a face, the face has to have a moderate level of fat. Look at pictures of the below beauties. They have a rather full face and this makes them look younger than their real age. Additionally, it gives off an impression that they look healthy and are even cheerful to others. This is the reason why people like and long for younger looking faces. But unfortunately, not everyone can have it. People with angular facial bones and/or having a lack of fat in their face cannot easily have such a well-rounded face shape.

How to get a round, younger looking face with volume?

With aging, the subcutaneous layer of fat shrinks and the skin loses firmness, and as a result, the facial soft tissues droop and wrinkles grow deep. Among the middle-aged, furrows deepen between the eyebrows and across the forehead, the lacrimal groove that runs diagonally, and deep smile lines and sunken cheeks make them look even older.
Young people aged between their 20s and 30s have firmness in their skin, but dents in their foreheads and temple areas resulted from underdeveloped facial bones and overdeveloped jaws make the face look more curved and older than their true age.
However, it is impossible to gain weight exclusively in the face. This is the reason why that the procedure to inject fillers in the desired parts and add volume to the face has achieved widespread appeal.

Injectable filler and autologous fat

What is injectable filler?
It is a soft filler that is injected into the skin to give it more volume. It includes hyaluronic acid filler and autologous fat.
A typical example of an autologous fat injection is transplantation of fat (autologous fat transfer). It uses excess fat from the abdomen, inner thighs and hips and Chinese patients especially favor this procedure. Pure fat cells are isolated from excess fat collected through centrifugation and then combined with PRP or stem cell extract to be injected into the desired part of the face. This autologous fat transfer is preferred because it is relatively simple and uses autologous fat, which sounds safer than artificial materials. But it is not always safe. In fact, there was a report about a patient suffering from an embolism (embolus travels through lymph or blood stream and blocks some or all blood vessels), which occurred during an autologous fat transfer at a plastic surgery center in Gangnam, Korea. In general, the needle used in fat transfer cannot go through the blood vessel because its tip is blunt. Nonetheless, there is a possibility that grafting fat cells or triglyceride passes through the damaged blood vessel wall at the location where fat is grafted.

What is more popular than autologous fat transfer is injectable dermal filler using artificial materials. The duration of dermal filler differs depending on the materials used. The most well-known type is hyaluronic acid filler and it lasts about 9-12 months. It binds water molecules in the body to expand and add volume to the desired parts. Plus, it could be always melted using hyalase, which is the most-used filler in the world. But it does not last long. Cleviel Hyaluronic Acid Filler of AESTURA of AMOREPACIFIC Group also uses hyalase.

In addition, there is a filler using calcium and it lasts twice longer than hyaluronic filler. Fillers made of PMMA (polymethlymetacrylate) like artefill of AESTURA have an advantage for having an over 10-year duration and a natural shape with autologous collagen formation, but this type is hard to remove. Regardless of type, injectable dermal fillers could cause the same symptoms as autologous fat transfers if it enters into the blood vessel.
  • (Left) Cleviel is a resorbable hyaluronic acid filler that was studied/developed by AMOREPACIFIC R&D Center. It has the highest content (50mg) in the world with a high level of elasticity and viscosity. It comes in three lines: Contour, Volume and Prime.
(Right) Artefill is a non-resorbable PMMA where PPMA particles in the filler stimulate collagen formation in the skin. An American aesthetics company named Suneva Medical supplies and AESTURA distributes it.

Both autologous fat transfers and injectable dermal fillers are widely used and to that extent, there are numerous case studies concerning side effects. If any of you are interested in either of them, I would like to recommend you to get help from specialists who are very experienced and know deeply about the anatomical structure of the site of surgery.
